• School Forms

  All students must have their immunization records (GA Form 3231) on file with the school. Students in grades pre-kindergarten through 11th must also have their Hearing, Vision, & Dental information (GA Form 3300) on file with the school. You can find all the required forms by grade level below. Please remember that these forms are to be updated yearly.

  We use 1st Day School Supplies as our school supply supplier. Click here to order the list for your grade level. If you would like to do your school shopping on your own, please reference the school supply lists organized by grade level on the 1st Day School Supplies website.

• Summer Reading

  Please use the links at the bottom of the page to view the grade-level summer reading and work requirements. Please note that these requirements apply to students in third through twelfth grade.

• Bus Services

  Notre Dame Academy offers both morning and afternoon bus transportation. Van Service will be provided on a first come, first served basis. Please register online by clicking here. Inquiries may be sent directly via email to Transportation@ndacademy.org.
